Care Planning Process

We believe passionately that conscientious care planning and embracing family is a crucial step towards excellent care. A designated leader will be in-charge of developing and evaluating your care plan. A template for care is already available as a guide before your care commences. You are strongly advised to include a family member or a supporter from the initial stages.

Stage 1

Free Consultation

A meeting convenient to you is set-up to enable us to learn about you, your needs and your expectations. We then work you through the solutions we may provide.

Stage 2

Consenting to your care plan

Selecting us as your approved agency warrants a follow-up from Stage 1, where more details about your care requirements are explored. The outcome of this stage is to generate a comprehensive care plan.

Stage 3

Developing your care plan

A personalized care plan is generated tailored to the information gathered from stages 1 & 2. A session is arranged for you to review the developed care plan where we can discuss necessary changes to your required care.

Stage 4

Your Care Commences

We are happy to begin your care plan once you are satisfied with the contents of care. We are also willing to introduce you to your Care Team Manager.

Stage 5

Evaluating your Care

We are passionate about tailoring your needs to your care plan, which will undergo a monthly review to effectively meet your ever-changing needs without comprising on quality of service.

Follow Up Care

You can choose which staff you prefer to work with, although we do appoint someone specifically to you based on their experience. This creates an atmosphere for a more friendly, stable and a trustful home environment
